Insurance Information

The deadline to waive/enroll for the Fall semester is September 1.

The deadline to waive/enroll for the Spring semester is February 1.

In order to attend Rockford University, international students are required to have adequate health insurance.

Currently, we have a mandatory with waiver system which means that the insurance charge remains on your student account until you have met the insurance requirement – either by enrolling in the student plan or providing proof of comparable insurance by completing the insurance waiver.  Waivers and enrollments must be submitted, by the deadline, your first semester at Rockford University and every Fall semester thereafter.

Rockford University is committed to ensuring the availability of a health care plan to all students.

The current student insurance is an accident/injury plan available to all international students taking classes at Rockford University.

In order to have the health insurance charge waived on your student account, you must complete the insurance waiver before the deadline the first semester you attend Rockford University and every Fall semester thereafter. Waivers submitted at any other time will be invalid and will not be accepted.

  • Individuals being tested should not eat, drink, use tobacco, brush their teeth, or use mouthwash 60 minutes prior to submitting a sample.
  • There is no minimum age requirement to participate in SHIELD testing.
  • All visitors must wear a mask as they enter and leave the testing site.

Pharmacy-Based Testing Sites

 

Pharmacies require appointments that can be scheduled online. There should not be an out of pocket payment for tests given to someone who is symptomatic, who is a close contact to someone who tested positive, or who is recommended to test by a healthcare provider. Do not pay a fee for a COVID-19 test.

Meet Our Staff

Kristen Clarke, MSW, LCSW
Director of Lang Wellness

Kristen Clarke received her BSW from University of St. Francis and MSW from Aurora University. She specializes in mood disorders including depression and bipolar, as well as anxiety disorders.  Kristen believes that college students are at a unique place in their lives that present new challenges.  Kristen works with students to sort out past issues and current stressors to help students navigate through the college years and prepare themselves for their future endeavors.

Jamil Hussain, MD
Physician

Dr. Hussain is an experienced and well-established Family Practice Physician in the Rockford Community.  His special interest in family practice, in addition to his high level of professionalism and care, greatly benefits Rockford University as our consulting physician.

Autumn Schaal LPC, NCC
Personal Counselor and Disability Support Services Coordinator

Autumn Schaal received her MA in Clinical Mental Health Counseling from Adams State University. Autumn’s areas of interest include: emotional growth, anxiety, and overcoming life stressors. She uses an integrated approach focusing on the whole person, incorporating mindfulness, physical, mental, and emotional well-being, to promote healing through the context of relationship and community. Her goal is to create a space in which students feel safe, accepted, and empowered to sustain positive and fulfilling relationships with both self and others.

Stephanie Garrison, MA, LPC, NCC
Personal Counselor

Stephanie received her MA in Clinical Mental Health Counseling from Judson University. Her areas of interest include: anxiety and depression, dialectical behavioral therapy, and trauma. Stephanie recognizes that college is a time of great change and development, and she believes in meeting students where they are at and helping them realize their potential by balancing their physical, social, emotional, intellectual and spiritual well-being.

 

Patty Schneider, RN, BSN
Nurse 

Patty received her BSN from NIU in 1989, and worked for 15 years on the Adult Medical/Oncology Unit at Swedish American Hospital. Patty enjoys assisting, educating and empowering students to make healthy choices. She believes that these choices directly impact their future health and vitality.

Laura Godin
Office Manager

Laura received her Bachelor of Science in Accounting from Rockford University.  Laura has previously worked in Student Administrative Services at Rockford University and in her role as Office Manager at Lang provides support to all of our students especially in the areas of record keeping, health insurance, both domestic and international, immunizations, and billing.

FREQUENTLY ASKED QUESTIONS

Who is eligible to receive services from Lang Wellness Center?

All students, faculty and staff may use the Health Center.

Do I need to make an appointment to receive services?

Lang Wellness Center is a counselor directed facility under the medical direction of Dr. Jamil Hussain. The health center is open Monday through Friday 10 a.m. to 4 p.m. (T, W, Th 10 a.m. to 2 p.m. during the summer) for nursing assessments, treatment and referrals, please call 815-226-4083 for an appointment. Appointments are required for counseling services and can be made via phone, e-mail or in person at the health center. Appointments should be scheduled for matters such as medical incompletes and withdrawals, medical meal plan exemptions, physician appointments and disability accommodations.

Is there a physician on staff at the health center?

Dr. Jamil Hussain is available to see students on Tuesday afternoons by appointment for physical exams and assessment/ treatment of illness or injury. All visits are by appointment only and require a prior assessment and referral by Lang Wellness Center nurses. If students are insured under the Rockford University student health plan, the health center staff will process your visit. If your health insurance is through any other carrier, your student account will be billed for the visit, and the insurance claim form will be filed with your insurance carrier for possible reimbursement.

Lang Wellness Center is located across from the Howard Colman Library and next to Cummings Complex.

What do I do in case of injury/illness after hours or in case of an emergency?

Students are encouraged to visit the health center during normal operating hours for an assessment and/or referral if they are feeling ill. If injury or illness occurs after hours, there are several local options available. When receiving services at any outside facility, please remember to take your insurance card!

Will other students, faculty, staff or my parents find out that I am receiving services at Lang Wellness Center?

All information and visits are CONFIDENTIAL. Information cannot be released or exchanged without written consent. Confidentiality may be waived only for instances in which an individual is a danger to oneself or others. All staff must report suspected child abuse/neglect in accordance with the Mandated Reporter Act.

What should I do if I am sick and must miss class?

All students should be aware of the attendance policy for each of their classes. The student is responsible for contacting the professor regarding the missed class(es) and any work that is missed. Lang Wellness Center will contact instructors to inform them of a student’s illness if the student is seen at the health center and is determined to be too ill to attend class, OR if Lang Wellness Center receives medical documentation from a physician regarding the illness or injury. Final decisions regarding missed classes and work are at the discretion of the individual instructor.

What does it mean if I have a “hold” on my account?

Illinois State Law and Lang Wellness Center require proof of immunizations. Students who fail to turn in immunization records will have a Health Center Hold applied to their student account which prevents the student from registering for their classes. HEALTH REQUIREMENTS

The health requirements for all students attending Rockford University are explained below. Please review this document for a snapshot of the important student requirements.

Personal Health History

The information provided on this form gives the staff of the health center an overview of your current health status as well as your past medical history. This information assists the heath center staff in providing the best possible health care to you.

Immunization Records

Illinois State Law requires that all students taking 6 or more credit hours provide immunization records that indicate immunity to the communicable disease of Diphtheria, pertussis, tetanus (with the most recent dose having been received within the past 10 years), measles, mumps and rubella. In addition, all students under the age of 22 are required to show proof of having at least one dose of meningococcal conjugate vaccine on or after 16 years of age. Learn more >>

In order to have the health insurance charge waived on your student account, you must complete the insurance waiver before the deadline the first semester you attend Rockford University and every Fall semester thereafter. Waivers submitted at any other time will be invalid and will not be accepted.

IMMUNIZATIONS AND NURSING SERVICES

Immunization Information

Illinois State law requires that all students taking 6 or more credit hours provide immunization records that indicate immunity to communicable disease. Currently students are required to provide documentation of:

  • Measles: (Rubeola) Two doses are required. The first must be administered on or after the first birthday and the second must be administered no sooner than four weeks after the first.
  • Mumps: Two doses are required. The first must be administered on or after the first birthday and the second must be administered no sooner than four weeks after the first.
  • Rubella: Two doses are required. The first must be administered on or after the first birthday and the second must be administered no sooner than four weeks after the first.
  • Tetanus/Diphtheria/Pertussis: Students shall provide dates of any combination of three or more doses of Diphtheria, Tetanus, and Pertussis containing vaccine. One dose must be a Tdap vaccine. The last dose of vaccine (DTP, DTaP, DT, Td, or Tdap) must have been received within 10 years prior to the term of current enrollment. The minimum time interval between the first and second dose must have been at least four weeks (28 days), with the third dose having been received at least six months after the second or last dose of the basic series. Receipt of Tetanus Toxoid (T.T.) vaccine is not acceptable in fulfilling this requirement.
  • Meningococcal Vaccine: All students under the age of 22 are required to show proof of having at least one dose of meningococcal conjugate vaccine on or after 16 years of age.                             Meningococcal Disease Brochure: What Parents and Students Need to Know

In lieu of these vaccinations for Measles, Mumps, and Rubella, students may submit titre (blood test) results that indicate immunity to these diseases. Remember to make sure that you have provided your immunization records to Lang Wellness.  If you have not, there will be a HOLD on your account which will stop your registration for future semesters. Please provide your records to us as soon as possible. If you have questions about whether or not you have fulfilled this requirement, please call or stop by.

 

The following immunizations are available at Lang Wellness Center at minimal cost:

  • Adacel Tdap/Td –$40
  • TB Skin Test — $10
  • 2-Step TB Skin Test — $15
  • MMR (Measles, Mumps, Rubella) — $85
  • Meningococcal — $115
  • Seasonal Flu — $20

Students who are exempt from providing this immunization information include:

  • For measles, mumps, and rubella vaccines only, in lieu of proof of immunity, students may provide evidence of birth on or before January 1, 1957, such as a birth certificate, drivers license, or personal identification card issued by the Secretary of State.
  • Students who have medical documentation from a physician stating they may not receive a vaccination due to a medical condition.
