Student Makeup Exam Form

  • Please read and acknowledge the instructions for requesting an exam carefully before submission.
  • Requests must be submitted for dates at least five (5) calendar business days after the form submission date.
  • All makeup exams must have the approval of the course instructor. Requests should indicate at least three (3) separate time preferences as scheduling options.
  • Clearly indicate first and last name, course name and number, and a working contact phone number for proper request processing.
  • CLS adheres to the Rockford University Academic Integrity Policy which applies to plagiarism, cheating, and the academic regulations of Computer Labs and Library services.   • Please review the 7 Simple Rules for Test Success in CLS.
  • Notifications will be forwarded exclusively to Rockford University email accounts. The faculty of record for courses listed on the student request form will be copied on student test scheduling confirmation email.

Modern Language Exam Request

Modern Language Placement Exam Information:

  • Testing will be held in the Colman Library, Main Floor. Check-in will be at the Circulation Desk.
  • Each student is required to adhere to CLS Testing Regulations and the RU Code of Academic Integrity.
  • On the day of testing, each test taker must present at least one current, valid form of identification that includes the test taker’s name and a recognizable photograph. Acceptable forms of identification include current and valid: driver’s license, state-approved or federal ID, Rockford University ID, or Passport.
  • Use of electronic devices of any kind are prohibited including during examination breaks. Devices include but are not limited to, cell phones, digital or smart watches, fitness bands, and any other device with recording, internet, or communication capabilities. All devices must be powered off and securely stored in a CLS Locker. If a student is found with any unapproved items after testing begins, the testing session will be terminated immediately.
  • Personal items of any kind, including but not limited to purses, computer bags, backpacks, and duffle bags are prohibited in testing spaces. All items must be stored securely in a CLS Locker.
  • Apparel, including jackets, hoodies, sweaters with pockets and/or hoods, coats, hats, and sunglasses are not permitted in the testing environment and need to be placed in a CLS Locker. Discretionary allowances may be made for religious apparel. All apparel is subject to inspection by the proctor.

Don’t Cancel That Class Request Form

Please carefully read the instructions for requesting a Don’t Cancel That Class (DCTC) presentation before submission.

  • Requests must be submitted for dates at least five (5) calendar business days after the form submission date to allow the CLS staff to make schedule adjustments.
  • Clearly indicate class, day of the week, location, and class time.
  • Please understand that the DCTC initiative is NOT a substitute that you can call in when you don’t feel well. This initiative is meant to be an alternative to canceling a class when you have an anticipated absence.
  • CLS reserves the right to refuse a visit request at any time.
  • CLS will notify you of acceptance/refusal of service within two (2) calendar business days of the form submission date.

TEAS Exam Testing Information:

  • At this time ATI TEAS exam scores from an online exam will not be accepted. TEAS exams must be taken in person by appointment only.
  • Testing will only be offered on dates listed on the Summer 2024 – How to Register to take the TEAS Exam.
  • Testing will be held in the Howard Colman Library, Main Floor. Check-in will be at the Circulation Desk.
  • The exam will begin promptly at the time designated on the exam schedule. Students must check in no later than 15 minutes before the exam start time to allow for the onboarding process.
  • On testing day, you are required to bring a valid ID (i.e. RU ID, driver’s license, passport) with a current photograph.
  • Use of electronic devices of any kind are prohibited including during examination breaks. Devices include but are not limited to, cell phones, digital or smart watches, fitness bands, and any other device with recording, internet, or communication capabilities. All devices must be powered off and stored outside of the testing environment. If a student is found with any unapproved items after testing begins, the testing session will be terminated immediately.
  • Personal items of any kind, including but not limited to purses, computer bags, backpacks, and duffle bags are prohibited in testing spaces. All items must be stored outside of the testing environment.
  • Apparel, including jackets, hoodies, sweaters, coats, hats, and sunglasses are not permitted in the testing environment. Discretionary allowances may be made for religious apparel. All apparel is subject to inspection by the proctor.
  • Food and drink of any kind are prohibited in the testing environment.

  • Dictionary use and the use of course books or notes are prohibited. Also, students may not browse the Internet or access any outside websites.
  • ATI log-in information: For the online TEAS test create a student account at https://www.atitesting.com/ before the test date and have your log-in information with you.
  • TEAS Exam results are valid for 6 months from the date the exam is taken.
  • Payment information: Before taking the exam, $65 is to be made payable to Rockford University SAS. Proof of payment is required before the administration of the exam. Rockford University Student Administrative Services (SAS) are located on the main level of Nelson Hall. SAS can be reached at 815.394.3700, email sas@rockford.edu. The cost of the TEAS exam will increase to $70 starting June 1, 2024.

Supplemental Instruction (SI)

Supplemental Instruction (SI) is an academic support program that targets historically difficult courses. SI sessions are regularly scheduled, out-of-class group review sessions for all students enrolled in a targeted course. The SI sessions are facilitated by SI Leaders who are trained to share with other students how to effectively study for the course and how to maximize their potential for academic success.

Peer Tutoring

Peer tutors provide learning and study strategies for their assigned students, while clarifying and reinforcing course content, assignments and materials. Peer tutoring is individualized according to the needs of the students seeking tutoring support.
