Student Employment Forms

These forms can be downloaded and filled out. A copy of the completed form should be brought to SAS.

Application for Student Employment

 
All interested current Rockford College undergraduate students must first complete the Application for Student Employment form. Once this is completed, submit it to SAS. A copy of the application will be given to the student to carry throughout the interview process.

Confidentiality Agreement

All student employees must complete the confidentiality form.  This form must be signed by both the student employee and a witness.

All student employees must file this form prior to beginning work at Rockford College. Complete section 1 of the I-9, then bring the I-9 to SAS, along with documentation described on page 3.

Open Position Notice

When a department has a position to post on the Student Employment On-Campus Jobs page, they must first complete this form and submit it to SAS. If the need arises to create an additional position within a department, the Open Position Form must first be approved by the appropriate Vice President.

Student Employee Evaluation
       

Evaluations fulfill several objectives:
• To provide each student with individual, constructive feedback on his/her work

   performance.
• To give the supervisor the opportunity to discuss areas for improved performance during

   the remainder of the year and to solicit additional tasks, responsibilities or project.
• To help students establish a documented work history.

Written Warning Notice
  

Conflicts and problems should first be corrected on an informal level. If students display poor work performance, tardiness, excessive absences or other unprofessional conduct, they will receive a written warning from their supervisor. The form is signed by both the supervisor and the student employee and then forwarded to SAS. Three written warnings must be given before termination, unless reasons for automatic termination applies.

Three written warnings must be given before termination, unless reasons for automatic termination applies. Reasons for automatic termination of student employment include, but are not limited to violation of confidentiality, dishonesty (including falsifying timesheets) and not showing up for work for three consecutive days. All terminations must be documented through the use of the Notice of Termination form.
