to the 2006-07 season! We're looking forward to another
great year of Regent Athletics.
In
order for any Rockford College student to participate
in Intercollegiate Athletics during the 2006-07 academic
years, the forms described below must be completed
in their entirety.
Please
print the appropriate forms as outlined below, fill
them out completely including all required signatures
and bring the completed forms TO THE FIRST
TEAM MEETING OF YOUR SPORT. (Contact the
head coach for information on the date and time of
the first team meeting for your sport). Or send them
to your coach or head athletic trainer so that they
are received prior to your first team meeting.
Student-athletes
(returning or new) will NOT be able
to participate in any activities (including pre-season
or any try-outs) without full and accurate completion
of ALL required forms. Please be sure to read through
all the information below to ensure you have all of
the necessary paperwork to bring to your first team
meeting. Please note that there are several changes
from last year, so please fill out every form completely.

and print them out for completion.
Both
Rockford College and the NCAA REQUIRE that
all student-athletes have health insurance. Please
note, all Rockford College student-athletes must provide
evidence of insurance that includes coverage for athletically-related
injuries. This is a pre-requisite for practice and
competition. No student will be allowed to participate
in any way until such evidence of current insurance
coverage is on file with the Rockford College department
of athletics. The enclosed DIII Insurance Information
Form and an insurance card, or photocopy of both
sides, must be on file before a student can participate.
Insurance
coverage must have a limit of at least $75,000 and
cover athletically-related injuries. If your insurance
does not meet these requirements, Rockford College
will review the individual circumstances to determine
if the insurance meets the insurance coverage requirement.
Rockford
College will assume no responsibility whatsoever for
the payment of, or authorization to pay, medical expenses
resulting from injuries that occur while participating
in intercollegiate athletics at Rockford College .
If you have questions regarding the terms of your
coverage, you should contact your insurer immediately.
Please be sure to note if there are any exclusions
in your policy regarding athletic related injuries.
The
NCAA's Catastrophic Injury Insurance Program covers
student-athletes who are catastrophically injured
while participating in a covered intercollegiate athletic
activity (subject to all policy terms and conditions).
The policy has a $75,000 deductible. This coverage
does not qualify as the basic coverage required for
participation in athletics at Rockford College . It
is supplemental coverage in the event of a catastrophic
injury. More information on this program can be found
on the NCAA's web-site at www.ncaa.org.

ALL students must complete the DIII
Insurance Information form. [Please note that
if you have an HMO (and some PPO's) you must designate
a Primary Care Physician within a 100-mile radius
of Rockford College. A parent or legal guardian must
sign this form to verify agreement with the health
insurance information, regardless of the age of the
involved student.]

ALL students must complete the Rockford
College Athlete
Emergency Information Card (this will be given
to your coach for away trips).
3.
ALL students must have one of the following
Rockford College Health History form completed:
A. Athletic
Health History Form
B. Returning Student-Athletes, same
as A for the 2006-2007 seasons.
4.
Any student who wishes to utilize a physical
from his/her own doctor in place of the team physical
that will be offered by Sport Link on August 10th
must have a form completed and signed by their physician
including address and phone number of physician.
5.
All students who utilize
the Rockford College Student health insurance plan
must understand that the insurance only covers athletics
to a $15,000 amount. All athletes are encourages
to get supplemental insurance that covers athletics.

Please note that if you are currently under a doctor's
care for an injury, a release from that doctor may
be required for you to begin participation in any
activities at Rockford College.
Student-athletes
need to be aware of the NCAA Banned Substances list
and should check with the Athletic Trainers for any
questions. CLICK
HERE for a list of NCAA Banned Substances. Student-athletes
who test positive are subject to at least a one-year
suspension and loss of eligibility.
Many
nutritional/dietary supplements contain NCAA banned
substances. In addition, the U.S. Food and Drug Administration
(FDA) does not strictly regulate the supplement industry.
Therefore, purity and safety of nutritional/dietary
supplements cannot be guaranteed. Impure supplements
may lead to a positive drug test. The use of supplements
is at the student-athlete's own risk. Student-athletes
should contact the Head Athletic Trainer Ashley Geiger
or Head Strength and Conditioning Coach Tom Kropf
for further information.
