Letter - Required Information
Immunizations
All full-time
students, including student-athletes, must meet Illinois
State Law and college admission requirements for immunizations
and TB screening. New students will receive admission
packets that explain these requirements. Returning
students should already have provided immunizations
records at their time of admission. You only need
to provide immunization records once! However, if
an immunization has expired, Lang Health Center will
give a booster prior to your athletic participation
for that year. All immunization records are to be
turned in and monitored by Lang Health Center.
Primary
Insurance Coverage
All full-time
students, including student-athletes, must have primary
health insurance. For those students who come to Rockford
College without insurance, the college provides student's
the opportunity to purchase a basic health insurance
plan at a group rate. Student-athletes should turn
in verification of their health insurance to Lang
Health Center and a copy of their insurance card to
the athletic trainer or their head coach.
Annual
Physical Exam
All student-athletes must have a physical
exam every year prior to their season in order to
be medically cleared for participation. For your convenience,
Rockford College hosts Sportlink, Inc., an outside
agency to provide physicals on campus preceding the
start of the school year. This year Sportlink will
be on campus Thursday August 10th . Cost of the physical
exam is $35 and includes the exam, screenings and
an echocardiogram. Cash or check is paid to Sportlink,
Inc. at time of exam. Athletes will be seen in the
following order:

volleyball, tennis, cross country and all other
athletes. |
Student-Athlete's
are strongly encouraged to take advantage of this
opportunity for a sport physical. However, if you
choose to utilize your own family physician for insurance
or other reasons, you are free to do so, as long as
a copy of your exam is turned into the Athletic Trainer
before your pre-season. If you forget to take your
physical at home and are not available on the 10th
, then you have the opportunity to schedule an exam
with Lang's family physician, Dr. Syed Mohiuddin.
The cost is $55 and does not include an echocardiogram.
Student-athletes should turn in their physical exams
to the athletic trainer or their head coach. The original
exam will be kept in Lang Health Center with your
permanent health file as long as you are a student
at Rockford College .
Athletes
Who Have Medical/Health Insurance:
- RC must have blue parent information form completed
with insurance information and a copy of insurance
card
- Must submit claim/bills to their insurance company
first
- Bring unpaid bills and explanation of benefits
to Head Athletic Trainer
- RC athletic insurance will consider remaining
charges including deductibles per the provision
of the intercollegiate athletic policy
- Athletes on IPA:
- RC must have blue parent information form completed
with insurance information
- Must submit claim/bill to IPA first, they may
pay bill.
- If they do not pay bill, bring bill and explanation
of benefits to Head Athletic Trainer to be submitted
to RC athletic insurance for consideration and payment
per the provisions of the intercollegiate athletic
policy
RC
Insurance for Athletes Does Not Cover:
- Preexisting conditions
- Illnesses
- Injuries sustained outside RC athletics
- Injuries sustained out of season
- Bills attained outside student/athlete's HMO
vendor
- Costs above and beyond usual and customary charges
- RC reserves the right to uphold denials from
primary insurance company
- Medical bills incurred without RC knowledge of
injury
