Medical Incomplete
The medical incomplete policy is designed for students who are
unable to complete course work and/or take the final exam due to
medical or psychological illness. A medical incomplete is to allow
students an extension of time to complete work missed due to illness
or extenuating circumstance (i.e. death in the family, illness of
a child, etc.) and fulfill the requirements of the class. Consideration
for a medical incomplete is on a course by course basis. If all
courses are involved, a medical withdrawal is recommended. A medical
incomplete taken for multiple courses in the same semester is not
recommended. The instructor will determine how the student makes
up the work in the class and may require the student to repeat the
class in its entirety if sufficient work has not been completed.
To receive a grade of medical incomplete a student must complete
an application and provide documentation of medical necessity to
Lang Center for Health, Wellness and Counseling. Medical necessity
must be documented in writing by the treating physician, psychologist
or counselor. During the application process the student must contact
the involved professor, the academic advisor and the Student Administration
Services representative. Academic, financial and administrative
issues must be addressed before the medical incomplete will be processed.
The instructor will determine what must be done to fulfill the course
requirements and attach this plan to the application. Finally, the
application must be signed by the academic dean.
For each course in which students receive an incomplete, the instructor
will submit a dual grade – IM/B, IM/C, IM/D, IM/F. If the
student does not complete the course requirements within six months
after the official end of the semester in which the medical incomplete
was granted, the default grade (second grade) will become the official
grade.
Students can receive an extension of time to remove a medical incomplete
with a recommendation from the professor and the advisor. Only one
extension (not to exceed six months) will be granted; the extension
must be arranged before the regular six month extension expires.
The extension form is available at Lang Center and must be submitted
to the professor, advisor and SAS for signature. If the work is
not completed by the end of the extension, the grade will become
the recorded default grade. It is the student’s responsibility
to initiate the request for an extension, make the necessary arrangements
and submit the paperwork. The extension also must be approved by
the academic dean..
The student is ultimately responsible for the medical incomplete
application process. When a student is medically disabled and unable
to complete the application process, Lang Center will intervene
and assist with processing until the student is able to continue
on his or her own.
The medical incomplete process is not finalized until the student
has a grade in the course. It is the student’s responsibility
to complete the work and earn the grade for the class. When the
coursework is completed the instructor will submit the grade on
a change of grade form to Student Administrative Services. The SAS
office will notify the professor, advisor, Lang and the Academic
Dean that the process has been completed and the grade has been
recorded.
Any exception to this policy must be submitted to the Academic
Dean in writing for consideration.
